Below we have provided some general information about our practice to help you prepare for you appointment with Dr. Mudd. If you have any additional questions, please feel free to call our office and a member of our staff will be happy to assist you.

*For the convenience of our patients, we are often able to schedule after-hours appointments on Thursdays and Fridays when necessary to accommodate individual patient needs.

Appointments

To schedule an appointment, please call the New You Center at (614) 888-6833 during office hours. Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule. New patients are always welcome.

To meet your expectations for excellent care, it is important that the time we reserve especially for you to work with Dr. Mudd is shaped around the description of your concerns. It is helpful to let our staff know as much as you can about your particular goals when scheduling your appointments. If this is your first visit, then you will be coming for a consultation with Dr. Mudd. Existing patients may schedule a consultation should you wish to discuss new treatment options.

Consultations

Our goal is to make every consultation as personalized and informative as possible. Dr. Mudd meets personally with you during your initial consultation to discuss your concerns in detail, review medical history, perform an assessment of the involved area(s), and discuss treatment options. Many of our patients find it helpful to bring a list of questions or concerns to their consultation. Family or friends may be present if you desire. You will also meet with a staff member to discuss scheduling and financial arrangements. For your convenience we accept cash, personal checks, debit cards, Visa, MasterCard, and Discover as methods of payment. Please feel free to contact our office staff if you should have any additional questions or concerns after you leave the office.

Scheduling for treatment/procedures may be done at the time of the consultation or at a later date per your preference.

If you are from out of town, we will try to accommodate your consultation and treatment on the same day, if possible. Certain procedures do require follow-up visits in order to attain the best results and we will work with you on scheduling these.

Cancellations

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. This courtesy on your part will make it possible to give your appointment to another patient.

Patient Registration Form

The following form can be downloaded to your computer and printed in advance of your consultation appointment. This will save you significant time at your visit. We suggest that you bring them at the time of your scheduled visit, but you may also mail them one week in advance prior to your appointment, if you would like.
